What it means when a drive is formatted for Mac, not Windows

This is a different situation from a drive that Windows never notices at all. Here, the drive shows up: it appears in File Explorer, sometimes even with a name and a size, but double-clicking it does nothing, or Windows offers to format it before you can use it. That's usually because the drive was formatted on a Mac, using a filesystem called APFS or HFS+, instead of the ones Windows understands, NTFS or exFAT.

Windows can tell that a drive is there and roughly how big it is, but it doesn't know how to read the specific way a Mac-formatted drive organizes its files, so it can't open what's actually stored on it. Nothing about the drive itself is damaged; the two systems are just speaking a different filesystem language.

Is my Mac-formatted drive broken if Windows won't open it?
No. This is a normal side effect of Windows and Mac organizing files in different ways. The drive itself, and what's stored on it, is very likely fine.
Should I format the drive so Windows can use it?
Not if you want to keep what's already on it. Formatting erases the drive's contents, and that's not something reading a Mac-formatted drive should require. It's worth confirming what's actually going on first.
